Full-time Remote Tele-Health Opportunity / Nephrologist MUST LIVE IN PHILADELPHIA, PA

Monogram Health  has a full-time (work-from-home)  telehealth opportunity for Nephrology candidates living in Philadelphia, Pennsylvania. MDs must be quality driven and have experience applying clinical expertise in a value-based, population health context.

Monogram Health  treats patients with polychronic diseases including CKD, and ESRD.  Monogram MDs are clinical leaders who contribute to the development and oversight of clinical strategies, policies, programs, processes, guidelines, and operations that drive improved patient health outcomes within a market. The MD oversees the delivery of care management services, ensuring that patient care is high quality, consistent and compliant. 

Roles and Responsibilities:

  • Responsibility for quality outcomes for large panels of health plan members in the Philadelphia area
  • Utilize data tools to report, monitor, and improve the quality  of core clinical KPI's - daily
  • Collaborate with a small POD of NPs
  • Conduct retrospective reviews of hospitalizations and dialysis treatment episodes to ensure Monogram's services are effectively rendered
  • Oversee the  continual refinement of clinical strategies, evidence-based clinical guidelines, and care management programs for effectiveness and efficiency within the Philadelphia market
  • Reduce unnecessary episodes of care, including hospital readmissions
  • Prepare clinical care reports in a timely fashion
  • Apply evidence-based criteria to determine the appropriateness of care 
  • Analyze data to build member care plans and ensure care goals are achieved
  • Achieve clinical goals and metrics in order to deliver improved health outcomes at a lower total cost of care

Position Requirements:

  • Must live in Philadelphia and be licensed in the state of Pennsylvania
  • Must be Board-certified in Nephrology
  • Must have experience with Medicaid, Medicare, quality improvement, medical utilization management, and risk adjustment
  • Must be willing to do in-home visits with NP 1 to 2 times a month / if needed
