FT Hospice and Palliative Care Telehealth Opportunity / Houston Texas

Monogram Health is seeking a Hospice Palliative Care physician for a full-time position in Houston Texas. Monogram serves patients with CKD, ESRD, and chronic conditions. MDs work from their home office. There is a 10% monthly travel requirement around the Houston area.

Roles and Responsibilities:

  • Remote (Work from home), Monday-Friday 8:30 am-5:30 pm
  • Treat and diagnose patients with CKD and Renal diseases
  • Serve as a collaborator with Nephrologist and other field clinicians 
  • Apply evidence-based criteria in determining the appropriateness and necessity of care
  • Identify and make practical, pointed interventions to improve member care

Requirements:

  • Must be board-certified in Internal Medicine, Hospice, or Palliative Care
  • MDs must be licensed in Texas and live in the Houston area. 
  • Must have experience in evidence-based clinical care
  • Must have experience in CKD, ESRD, and chronic care
  • Must have experience with quality improvement, risk adjustment, and medical utilization management
  • Must have strong management skills
