Join Froedtert ThedaCare's Established Geriatric Team in Clintonville and New London, WI

ThedaCare, Wisconsin's largest non-profit healthcare system, is seeking a Senior Care Physician to join our established team in Clintonville and New London region. You will be responsible for working with PALTC patients in community living facilities, developing relationships while administering high-quality patient care. 

Position Details: 

• Full-time (4-10s) or Part-time options available with flexible schedules!
• Location: Clintonville and New London region
• The team consists of 6 Physicians and 10 APCs
• 2-3 SNFs a day (patient schedule built within EPIC)
• On-Call by phone only: 1:11
• 4 weekends per year, Physicians cover holidays 1 per year, back up call by week (just for support for APC if needed), no going in when on call
• Patient census: 8-12 
• Support staff includes RNs and schedulers who help to coordinate provider visits, communicate with facilities, and triage phone calls.
• 27 Facilities total between skilled nursing and assisted living facilities
• Large opportunities for growth, such as Medical Director roles in the nursing homes that we serve

Will accept Internal Medicine, Family Medicine, or Geriatric trained physicians. 

• Visits completed at patient’s location (Nursing Home or Assisted Living)
• Acute visits, compliance visits, & discharge visits for short and long-term residents
• Advance Care Planning & Goals of Care discussions
• Minor procedures at bedside

Clintonville is situated in northeastern Waupaca County along US Route 45, roughly an hour's drive from Green Bay and Appleton.
