Join an expanding geriatric focused service line that provides mobile primary care services to multiple sub-acute, assisted livings, and long term care locations within a 60 mile radius of St. Cloud. In collaboration with our 4 current physicians and 2 advanced practice providers (will be expanding to 6) you will support our advanced practice providers in their onsite geriatric primary care practice for our long term care patients by completing the physician required regulatory visits, rounding in our subacute settings, and help support our APPs with MD required services in our assisted living facilities, This role is supported by nursing and clinic assistants.
This position will provide you with flexibility and independence in your practice while feeling supported by a care team that enables our doctors to have the time to provide outstanding care to our patients. Provide comprehensive evaluation, consultation, and treatment services to geriatric patients in a variety of settings. You would provide coverage to facilities located in the communities of: Long Prairie, Melrose, Sauk Centre, Paynesville, Cold Spring, Watkins, and Belgrade with shared coverage of our sub-acute facilities in St. Cloud.
