Atrium Health Transition Clinic - Kenilworth is an outpatient transition services clinic that is committed to helping our patients restore their body and mind. Our Transition Clinic is a multidisciplinary ambulatory Internal Medicine clinic for patients at high risk of complications and readmissions due to medical complexity or barriers to healthcare access. We offer integrated visits with provider, pharmacist, and social worker. During our 30 day follow up period, we identify and alleviate barriers to health focusing on patient engagement, access, and education. Care is delivered in office, virtually and/or with community paramedicine support based on patient’s needs. 
 
  • 100% Outpatient
  • Full-Time
  • Join the team of 1 Physician & 1 APP 
  • Office hours Monday-Friday (Flex 7am-5pm) 
  • Call rotation: Shared coverage rotated a week at a time; phone only
  • No Nights/Weekends
  • EPIC EMR
  • Be part of an integrated nationally recognized organization with physician-led medical group and embedded service line
  • Leverage access to DAX Copilot, an AI-powered clinical documentation tool integrated with Epic, streamlining clinical workflows by generating draft patient notes for efficient review, editing, and signature
  • Experience an environment that provides safe and equitable care for all patients
Training and/or Experience Required
  • Board-Certified Internal Medicine Physician
  • Interest in short-term management of complex patients - 18 years old and up
  • Passion for providing quality patient care in a highly productive clinic setting where collegiality and collaboration are a focus
  • Experience preferred

For 30 days following your release from the hospital, our dedicated team of doctors, pharmacists and social workers will provide the care and support you need. Our team will work with you to provide the resources, tools and education to help you have the safest, quickest and healthiest recovery possible. We can provide transitional care in our office, virtually or at your home, depending on your needs and available resources.

  • Connecting patients and caregivers with home health services
