Location: CentraCare – Coordinated Care Clinic, 3701 12th St N Ste 102, St. Cloud, MN 56303
FTE: Flexible, 0.4–1.0
In this position, you will co-lead the Coordinated Care Clinic and Correctional Care program. Leadership of these programs will provide you with a unique and rewarding opportunity to care for patients with significant social drivers of health. This role will partner with law enforcement, courts, and community agencies to improve care delivery and community outcomes.
What You’ll Do
Must be BE/BC in Family Medicine or Internal Medicine. Experience working within Addiction Medicine required. Must be able to obtain a MN License.
