Family Medicine Physician – Coordinated & Correctional Care Medical Director – St. Cloud, MN

This position is eligible for hiring incentives to include sign-on bonus and relocation assistance.

Summary

This position blends direct patient care with program leadership across coordinated care and correctional settings, offering the opportunity to collaborate closely with interdisciplinary teams and CentraCare leaders to shape strategy, improve care delivery, and impact outcomes for patients with complex medical, behavioral health, and social needs.

Schedule

• Flexible FTE (0.4–1.0) | Onsite – CentraCare Midsota- St. Cloud, MN
• Day shifts | Flexible hours between 8am-5pm
• No call, holiday or weekend requirements

Pay and Benefits

• Salary range: $260,000 - $300,000 per year
  • Pay may be determined by factors such as education, experience, skills, knowledge, location, and internal equity
  • Salary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rate
  • May be eligible for additional pay through bonuses, quality incentives, or production-based compensation.

Qualifications

Required:

• BE/BC in Family Medicine or Internal Medicine
• Experience working within Addiction Medicine
• Eligibility for Minnesota medical licensure

Preferred:

• Leadership experience in complex or vulnerable patient populations
• Interest in community health, population health, and systems-based care
