Emplify Health by Bellin is excited to announce an opportunity for a Physician to join the esteemed Escanaba and Iron Mountain, Michigan locations. This position offers a chance to be part of a dedicated team committed to providing exceptional healthcare to the community.

Opportunity Details: 

The?My Home My Care?program collaborates with patients in select nursing homes and assisted living facilities across the Escanaba, Green Bay, Iron Mountain, and Peshtigo regions to effectively manage patient care. Our program operates similarly to a mobile clinic, with a dedicated team of providers who travel to our partner facilities. They deliver transitional, acute, and routine care, ensuring that patients receive the same quality of service as they would in a traditional clinic, but without the need for travel. 

While our primary focus is on nursing homes and assisted living facilities, we are open to evaluating private home visits on a case-by-case basis in the Escanaba, Iron Mountain, and Peshtigo areas. The?My Home My Care?team possesses extensive experience in managing chronic conditions within the unique complexities of these environments. They collaborate closely with the broader care team to adopt a proactive approach to patient health, delivering care directly on-site. 

Shift Hours:

Full-Time 1.00 FTE 

Agreement Details:

• 2-year compensation guarantee, plus:
• Time off: 200 hrs./yr.; 264 hrs./yr. once compensation guarantee expires
• CME: $5,000 /yr. & 80 hrs. of PTO
• Call rotation: 1:12 weeks (2wk blocks)
• No weekend or holidays
• Relocation Assistance: Up to $12,000
• Health, Vision, Dental, Short- and Long-Term Disability, Life Insurance, Retirement Plans (401k and 457b), and Professional Liability Insurance provided by Bellin
