Outpatient IM/Intensivist opportunities - Greer Transitions Clinic in Charleston SC

Seeking fulltime and part-time intensivists to provide primary care for critically/chronically ill patients after ED discharge through the Roper St. Francis Physician Partners Transitions Clinic locations. 

• Employed position with Roper St. Francis Physician Partners Primary Care
• Salary-based compensation model
• Fulltime: Four days in clinic, one Admin day weekly
• Part-time: Two days in clinic weekly
• North Charleston and West Ashley locations  
• Full benefits and retirement
• Epic EMR
• PSLF qualified (private, not for profit system) 
• Patients are insured, underinsured and uninsured. Resources and charity funding available for patients through the Roper St. Francis Healthcare Foundation 

 The Greer Transitions Clinic improves care coordination as a one-stop-shop where underfunded patients with no primary medical home can visit with board-certified physicians, receive preventative care, connect with social services, receive financial assistance, transportation subsidies, birthing classes, food, clothing and so much more. The clinic has been transformational to the residents of North Charleston, seeing nearly 2,000 patients a year and reducing emergency department visits for patients of the clinic. Learn more here!
