Position summary

The Lead Physician provides comprehensive primary care to an assigned panel of members and functions as a key clinical leader within the center. In close partnership with the Medical Director, the Lead Physician supports clinicians in delivering high quality, value based care and ensures that interdisciplinary teams operate effectively. This role offers real time clinical guidance to physicians, advanced practice providers, nurses, medical assistants, and behavioral health specialists, and plays a central role in mentoring, developing, and supporting providers across the care continuum. In addition to direct patient care—representing approximately 70–80% of the role—the Lead Physician helps drive the center’s quality, utilization, and engagement goals, contributes to operational planning, and represents AbsoluteCare in relationships with community partners, academic programs, hospitals, and payer organizations. The position requires strong clinical judgment, leadership skill, and a commitment to improving outcomes for medically complex and underserved populations.

Responsibilities

  • Provide high quality primary care to a panel of members, managing chronic diseases, acute conditions, and preventive needs.
  • Lead the pod’s interdisciplinary care team and ensure coordinated, whole person care grounded in evidence based practice.
  • Offer real time clinical support and guidance to physicians, APPs, nurses, MAs, behavioral health staff, and other team members.
  • Coach, mentor, and develop clinicians in primary care, urgent care, and community based care delivery; support them in effectively leading their own interdisciplinary teams.
  • Participate actively in population health huddles, guiding teams toward achieving quality, utilization, and preventive care benchmarks.
  • Manage acute exacerbations of chronic illnesses onsite and determine when escalation to higher levels of care is required.
  • Accept same day, next day, and urgent appointments, including members recently discharged from hospitals, ERs, or skilled nursing facilities, to prevent readmissions and ensure stabilized transitions of care.
  • Track member status across the entire care continuum—hospitalizations, ER visits, SNF stays, home based care—and intervene early to reduce preventable utilization.
  • Collaborate with social work, care management, nursing, pharmacy, nutrition, and behavioral health to build and implement comprehensive plans of care.
  • Monitor, track, and achieve key quality metrics and outcomes tied to value based care performance.
  • Participate in leadership meetings with the Medical Director and help shape clinical operations, workflows, and strategic initiatives.
  • Serve as an AbsoluteCare representative to community based organizations, academic partners, hospital systems, and payer groups.
  • Contribute to the development of clinical protocols, care pathways, and upskilling initiatives that support top of license practice across all clinical roles.
  • Utilize the electronic health record at full capacity to support documentation, quality efforts, patient tracking, and workflow efficiency.
  • Participate in multidisciplinary case reviews and recommend treatment strategies to resolve ongoing clinical issues.
  • Provide on call coverage in rotation with other providers.
  • Perform additional duties consistent with chronic care, complex care management, and the needs of the center.

Qualifications

  • Active, unrestricted medical license in the state of employment.
  • At least 5 years of clinical experience, including leadership of interdisciplinary teams.
  • Experience in primary care, inpatient medicine, and/or community based care settings serving complex populations.
  • Proven ability to train, mentor, and support physicians and advanced practice providers.
  • Strong interpersonal communication, critical thinking, and problem solving skills.
  • Ability to manage multiple priorities, lead initiatives, and contribute to operational and clinical improvement.
  • Independent, collaborative, and mission driven, with demonstrated interest in population health, utilization management, and continuous development of a value based care model.
