M Health Fairview is seeking a Family Medicine Physician to join our well-established primary care practice at M Health Fairview Clinic Burnsville. This opportunity offers a collegial environment, strong specialty support, and the ability to focus on adult patients only.

Practice Details:

• 15-provider primary care practice
• Adult patient panel only
• Average of 18 patients per day
• FTE = 34 patient contact hours per week
• Openings: 0.75 to 1.0 FTE (Full Time) - Flexible schedule
• Shared call rotation: 1:23
• As part of Fairview s Accountable Care Organization (ACO), providers practice in a collaborative, team-based model focused on quality outcomes, care coordination, and population health management.
• Fully integrated EPIC electronic medical record
• Multidisciplinary team-based care model
• M Health Fairview Clinic Burnsville is adjacent to Fairview Ridges Hospital.

Qualifications:

• Graduate of an accredited ACGME/AOA Family Medicine residency training program.
• Board Certified/Board Prepared with the American Board of Medical Specialties or American Osteopathic Association.
• Licensed or ability to obtain Minnesota Board of Medical Practice Licensure.
• Ability to obtain and /or maintain DEA certification for the State of Minnesota.
• Meet threshold criteria/qualifications for Credentialing and Privileges.

Qualifications:

$281,854 - $313,172 Annual *based on full time (1.0 FTE)
