Physician - Family Medicine

• Specialty:  Family Practice - Primary Care
• Schedule: 1.0 FTE, with 36 Patient Contact Hours; 4 hours of admin time per week. Clinic hours are 7:00am to 5:00pm. Providers work +2 hours a week (outside of 8:00am to 5:00pm).
• Call Schedule: On-Call provider takes nightly/weekend/holiday call, 1:7.
• Practice Detail: Ascension Medical Group in Chilton, Wisconsin is looking for a Family Practice Physician to join our elite team (2 MDs and 5 Nurse Practitioners-fully staffed). Our clinic is located on the campus of Ascension-Calumet Hospital.

Ascension-Wisconsin is looking for a Board Certified/Board Eligible Physician for our Primary Care-Family Medicine Team.

Responsibilities: 

• Provide medical services to patients in an assigned area.
• Evaluate and treat patients with appropriate medical diagnostic and treatment skills. 
• Recommends, participates in, and works to ensure the success of efforts to improve cost effectiveness and quality of care provided to patients.
• Participate in academic programs (e.g. medical education and research) as requested.
• Provide appropriate documentation that meets insurance company requirements; appeal all denials in a timely manner.
• Opportunity to participate in inpatient and outpatient consultation for specialty, which may include on call schedules.

St Elizabeth Hospital is newly renovated with a state of the art 90-bed patient tower with a full complement of sub-specialists on staff (Cardiology, Pulmonary/Critical Care, Gastroenterology, Renal and Surgical Team).

Licensure / Certification / Registration:

• One or more of the following required:
