Lead care the way it was meant to be.

Most leadership roles pull you away from patients — or bury you in metrics that don’t matter. At Gather Health, we built something different on purpose.

We’re a Social Primary Care organization serving clinically and socially complex older adults in underserved Boston communities. Here, you’ll lead clinical teams, shaping care delivery, and building something that lasts.

This is a clinical leadership role with real impact. Full stop.

What makes this role different

Lead clinicians — not just operations
You’ll mentor and develop Primary Care Providers across multiple clinics, supporting them in delivering relationship-based, whole-person care. You’re not managing from a distance — you’re in it with them.

Stay connected to patients
This is predominantly a leadership role with a clinical care component, where you will maintain a small patient panel to stay closely connected to the patient experience.

A care model designed for outcomes — not volume
We operate in a value-based model. That means your leadership is focused on quality, access, and outcomes — not throughput or visit counts.

Build and scale something meaningful
We’re growing. You’ll partner with senior leadership to open new clinics, develop teams, and scale a care model designed specifically for older adults with complex needs.

A real care team behind every clinician
Our model surrounds providers with support — including Gather Guides, Medical Technician Partners (EMTs/Paramedics), and integrated operations partners — so your teams can focus on care, not chaos.

What you’ll do

  • Carry a small patient panel while providing clinical leadership across multiple care centers in the Boston market
  • Lead day-to-day clinical operations, including patient flow, care delivery, and team workflows
  • Mentor, develop, and performance-manage Primary Care Providers and clinical staff through coaching, case discussions, and real-time problem solving
  • Drive clinical quality outcomes while improving patient experience and implementing continuous quality improvement initiatives
  • Lead clinical huddles and foster strong interdisciplinary collaboration across care teams
  • Advance utilization management strategies to reduce avoidable ER visits and hospital admissions
  • Partner with leadership to execute growth strategy, including new clinic openings and expansion efforts
  • Support hiring, onboarding, and training of clinical teams across the region
  • Collaborate with operations, growth, and community teams to improve patient access, engagement, and outcomes
  • Use data, supported by our Gather Connect technology to continuously improve care delivery and performance

Our mission
We exist to partner with patients in building their best lives. We believe true health means treating the whole person — clinically, socially, and holistically.

Our goal is to create generational health and equity for communities across the country, one patient at a time.

What we’re looking for

Required:

  • Board Certified in Internal Medicine, Family Medicine, or Geriatrics (MD/DO)
  • Active Massachusetts medical license, or ability to obtain prior to start
  • 7+ years of clinical experience in adult, family, or geriatric care
  • 3+ years of clinical leadership experience
  • Experience in value-based care models
  • Strong clinical judgment and proficiency in evidence-based primary care
  • Ability to lead, coach, and inspire teams in a collaborative, low-hierarchy environment
  • Comfort working with the latest care delivery technology platforms
  • Strong ethics and ability to navigate complex, sensitive situations
