The Albany Medical College Division of General Medicine and Geriatrics is recruiting a board eligible/board certified geriatrician to join the faculty as a clinician educator for fellows and internal medicine residents. Our practice consists of an active homecare practice serving 11 counties in Northeastern New York and an outpatient internal medicine primary care teaching division which is the outpatient continuity training site for the geriatrics fellowship and half of our internal medicine training program. The practice is also a teaching site for medical students in years 1-4 and physician assistant and pharmacy students.
Our geriatric fellowship program is a unique academic and community partnership between Albany Medical College and the Eddy Senior Care system of St. Peter’s Health Partners, with inter-professional training sites in a variety of care settings.
The ideal candidate will have the opportunity to combine a part-time home visit practice with outpatient consultations and a Geriatric primary care panel. The role includes precepting of our Geriatrics fellow, residents, and medical students. Academic appointments commensurate with experience/qualifications.
Position Highlights:
• Provide comprehensive evaluation, diagnosis, and treatment of adult patients with a focus on geriatric medicine.
• Manage acute and chronic medical conditions commonly affecting older adults.
• Conduct geriatric assessments, including evaluation of cognition, functional status, mobility, fall risk, medication management, and social determinants of health.
• Develop individualized, patient-centered care plans that promote quality of life, independence, and healthy aging.
• Collaborate with primary care providers, specialists, nursing staff, therapists, social workers, and caregivers to coordinate care.
• Lead discussions regarding advance care planning, goals of care, and end-of-life decision-making when appropriate.
• Optimize medication regimens and address polypharmacy concerns.
• Provide preventive care and health maintenance services tailored to older adults.
• Participate in quality improvement, patient safety, and population health initiatives.
• Supervise and collaborate with advanced practice providers, residents, fellows, and medical students, as applicable.
• Maintain accurate and timely documentation in accordance with organizational, regulatory, and billing requirements.
• Participate in call coverage and departmental activities as assigned.
