In this role, you will care for Medicare Advantage members with complex medical, functional, and social needs — many of whom are older, medically fragile, and at high risk for avoidable complications.

As a Field-Based Staff Clinician in the New Jersey Region, you will be accountable for the health outcomes of a defined panel of members. You will meet patients where they are, understand the realities of their home environment, identify risks early, and help coordinate the right care at the right time. You will have the autonomy to practice thoughtfully, the support of an interdisciplinary team, and the technology to act with greater insight and precision.
