Physician Lead – Home-Based Primary Care - Outcomes Focused (Tampa–Orlando, FL)

HarmonyCares is seeking a Physician Lead for our home-based primary care team serving patients along the I-4 corridor (Tampa–Orlando).
This is a full-time, in-home primary care role designed for physicians who want to stay clinically active while also serving as a clinical leader—mentoring APPs, influencing care delivery, and driving team performance.
The Role
- ~40–50% protected leadership time
- Balance of clinical care + team leadership
- Smaller patient panel with medically complex seniors
- Typically 6–10 patients/day, 30–60 minute visits
- No clinic setting – care delivered in patients’ homes
You’ll operate within a dyad leadership model, partnering with operations leadership to drive provider performance, patient outcomes, and care quality.
Key Responsibilities
- Supervise and mentor NPs/PAs (collaborating physician role)
- Lead case reviews, ride-alongs, and clinical development
- Conduct chart audits and support quality initiatives (HEDIS)
- Participate in complex patient care and high-risk case discussions
- Partner with leadership on performance, productivity, and care strategy
Qualifications
- MD or DO
- Board Certified or Board Eligible (required)
- Active (or ability to obtain) Florida Medical license
- Valid driver’s license
- Comfortable with home-based care delivery
