Physician Lead | York County-Based Regional Leadership Role | $50K Retention Bonus

HarmonyCares is seeking a Physician Lead (Medical Pod Leader) based in York County to help develop and lead a growing home-based primary care team across Central and Eastern Pennsylvania.

The broader clinical pod will include providers serving patients in York, Dauphin, Lancaster, Berks/Reading, Northampton, and Lehigh counties. Each provider will care for a geographically concentrated patient panel within their own local area, while the Physician Lead provides clinical guidance and support across the regional team.

Our value-based model is designed around patient outcomes rather than visit volume. Physicians and APPs typically see 5–9 patients per day, allowing time to manage chronic conditions, coordinate services, and develop meaningful relationships with patients and families.

Providers are supported by an interdisciplinary team that includes nurses, social workers, care coordinators, and other clinicians. Patient panels are established through health plan partnerships, so there is no patient acquisition required and no hospital rounding.

The Physician Lead Role

This position combines direct patient care with significant clinical leadership responsibilities. Up to 50% of your time may be dedicated to mentoring and supervising Advanced Practice Providers, promoting clinical quality, and helping the regional practice grow.

You will remain clinically active by maintaining a smaller patient panel in the York area while serving as a clinical resource and leader for providers throughout the broader pod.

Qualifications

• MD or DO with an active Pennsylvania medical license
• Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred
• Experience collaborating with or supervising Nurse Practitioners or Physician Assistants strongly preferred
• Strong clinical experience caring for medically complex adult patients
• Interest in clinical leadership, value-based care, and relationship-centered medicine
